Taxes

All monthly membership fees are subject to applicable taxes.

Membership Fees & Billing

Membership fees are billed monthly and will be automatically renewed until cancelled in accordance with the cancellation policy.

Minimum Membership Commitment

All Memberships require a minimum commitment of six (6) months from the date of enrollment. This membership is designed to provide long-term value and consistent care, and is not intended for one-time or short-term use.

Treatments & Membership Benefits

As an MDTL Member, you may select one (1) treatment per month. Treatment selection may be discussed with our Medical Aesthetician or Nurse Injector to ensure appropriateness based on individual skin needs.

All treatments and benefits have no cash value, are non-transferable, and may only be redeemed while the membership is active.

Early Cancellation & Value Adjustment

If a membership is cancelled before completion of the six (6) month minimum term, any treatments or benefits already redeemed will be charged at their full regular retail value, less any membership fees paid to date. Any remaining balance must be paid before cancellation is finalized.

Refund Policy

All membership fees and purchases are non-refundable, including unused or partially used services.

Cancellation Policy

A 30-day written notice is required to cancel a membership. Cancellation requests submitted before the completion of the minimum term will be subject to the Early Cancellation & Value Adjustment policy stated above.

Roll-Over Policy

If a member is unable to use their monthly treatment, they may request a Roll-Over with sufficient advance notice. Rolled-over services must be used within 60 days of the original billing date.

Roll-Over requests may not be applied to consecutive months.

Membership Freeze

Members may request one (1) membership freeze per calendar year.

A $50 freeze fee applies unless the freeze is required due to extenuating circumstances (serious illness, military service, employment-related temporary relocation, etc.).

Freeze duration may range from one month up to one-half of the total membership term.

All freeze requests require 15 days' notice.

Program Intent

Memberships are structured to reward ongoing commitment and consistent treatment planning. Membership pricing reflects this value and is not intended to replace single-service pricing.
